WIOA Pre-Intake Form
For adults and dislocated workers seeking job training or career services
This is the first step toward WIOA services. Tell us about yourself, your work history, your education, and your career goals. A WIOA Career Coach uses your answers to start your application and review your eligibility for training and support services.
Maritime Pre-Training Form
For participants selected for Maritime Customized Training
Employers choose who attends Maritime Customized Training. If an employer has selected you, complete this form before training begins. It gathers the information needed for your WIOA application and eligibility form, so your WIOA Career Coach can finish your paperwork faster and you can focus on training.
ITA Application
For WIOA participants who need help paying for training
An Individual Training Account (ITA) can help pay for approved training programs after all other financial aid is applied. Tell us about the training you want, your enrollment status, and other aid you receive. Your WIOA Career Coach uses your answers to prepare your ITA application for review.
